EXTERNAL RHINOPLASTY: This patient had a prominent nasal dorsum and tip. She underwent hump reduction and osteotomies, tip narrowing and tip retrodisplacement (ie moving tip closer to her face) by resecting a segment of her tip cartilages. ONE YEAR POST OP.

EXTERNAL RHINOPLASTY - REVISION CASE WITH COMPLEX TIP RECONSTRUCTION. This patient was troubled with a distorted tubular tip with significant alar retraction after a previous rhinoplasty. She required reinforcement of her tip cartilages using septal cartilage extending out to the side to create a more uniform tip. This also helped displace the alar rim downwards. 9 MONTHS POST OP.

ENDONASAL RHINOPLASTY: This patient underwent bony and cartilaginous hump reduction with osteotomies as well as refinement of the tip and rotation of the tip upwards and elevation of the columella (base of nose) by shaving septal cartilage. ONE YEAR POST OP.
ENDONASAL RHINOPLASTY: This patient simply underwent nasal hump reduction, removing the bone and cartilage contributing to her hump (bump) on profile. The columella was also raised slightly. No modification of the tip was performed.
ENDONASAL RHINOPLASTY: This patient had nasal hump and broad nasal tip. She underwent removal of bone and cartilage from hump with oseotomies. Tip was actually very strong and wide and cartilages were asymmetric. She required not only partial resection of cartilage but advancement and suturing of cartilage to narrow tip ie lateral crural steal technique.
ENDONASAL RHINOPLASTY: This patient had nasal hump and broad nasal tip. She underwent removal of bone and cartilage from hump with oseotomies. Tip was actually very strong and wide and cartilages were asymmetric. She required not only partial resection of cartilage but advancement and suturing of cartilage to narrow tip ie lateral crural steal technique.
ENDONASAL RHINOPLASTY: This patient demonstrates the most common request in rhinoplasty. She underwent removal of the hump, both cartilage and bone. The tip was also made somewhat sharper. On the frontal view before surgery, the top of the nose is too narrow relative to the bottom. A more balanced frontal view was achieved with surgery.
ENDONASAL RHINOPLASTY: This patient demonstrates the most common request in rhinoplasty. She underwent removal of the hump, both cartilage and bone. The tip was also made somewhat sharper. On the frontal view before surgery, the top of the nose is too narrow relative to the bottom. A more balanced frontal view was achieved with surgery.
EXTERNAL RHINOPLASTY: This patient had a very prominent bony and cartilage hump deformity and over projected nasal tip. The hump was lowered and the entire nose including tip was deprojected (moved closer to the face). This is best done with multiple techniques using an external rhinoplasty approach. The nasal tip was also refined.
RHINOPLASTY: This patient simply underwent removal of nasal hump. No tip refinement was done but one can see very significant improvement with this simple procedure. 2 MONTHS POST OP.
ENDONASAL RHINOPLASTY: This patient simply underwent bony and cartilage hump reduction without tip work.
EXTERNAL RHINOPLASTY: This patient was bothered by an asymmetric wide tip with cleft in columella on frontal view and somewhat square shaped tip on profile. 8 MONTHS POST OP.
EXTERNAL RHINOPLASTY: Surgery involved complete resection of 3 mm of lateral crus and a septal shave to help rotate the tip upwards and improve tip contour as well as a number of sutures to narrow the tip and a morselized cartilage graft to fill in the cleft in the columella. 8 MONTHS POST OP.
EXTERNAL RHINOPLASTY: This patient had a bulbous tip nasal hump and over projected nasal dorsum and tip and asymmetric dorsum on frontal view. She underwent bony and cartilaginous hump reduction and osteotomies to address the nasal hump and asymmetry. This was combined with resection of the tip cartilages to lessen tip projection. In addition she underwent tip resection and suture techniques followed by a tip graft to improve tip definition further. 14 MONTHS POST OP.
EXTERNAL SEPTORHINOPLASTY: Patient required extensive surgery not only to lower the nasal hump but also lessen the hanging columella at the bottom of the nose and rotate the nose upwards. This involved hump reduction & osteotomies as well as tip suturing, resection of the septum to allow elevation of the columella and upward rotation of the tip. There is still a subtle swelling over the nasal hump but this is compressible i.e. soft tissue swelling and will resolve over the next several months. 4 MONTHS POST OP.
EXTERNAL RHINOPLASTY - REVISION CASE. This patient was seen for consideration of revision rhinoplasty both for aesthetics and breathing issues. She had collapse of the external nasal valve leading to breathing issues which was reconstructed with ear cartilage. The tip was brought closer to the face i.e. lessening tip projection. As well there was considerable tip asymmetry which was rectified with cartilage grafts. 1 YEAR POST OP.

EXTERNAL RHINOPLASTY: This patient underwent external rhinoplasty to lower the nasal hump (mostly cartilage) and also to improve tip definition/refinement. This involved not only partial resection and suturing of the tip cartilages but also placement of cartilage graft to the tip. 3 WEEKS POST OP.
EXTERNAL RHINOPLASTY: Significant change can be seen only 3 weeks following surgery and one can expect continued improvement in tip definition over the next year. One can appreciate the improved definition better on the view where she is smiling which leads to slightly more tension on the underlying tip cartilages and graft. 3 WEEKS POST OP.
EXTERNAL RHINOPLASTY: This patient underwent lowering of nasal bridge and slight refinement of tip including tip sutures to correct asymmetric tip. Also elevated columellar – labial angle. 5 MONTHS POST OP

EXTERNAL RHINOPLASTY WITH TIP GRAFT: This patient underwent aesthetic rhinoplasty primarily hump reduction 20 years prior to this procedure. What concerned her most was a somewhat widened drooping tip and to a lesser degree convexity of the dorsum. A segment of the lateral crus on each side was removed to allow upward rotation of the tip. 5 MONTHS POST OP.
EXTERNAL RHINOPLASTY WITH TIP GRAFT: To obtain further definition of the tip, a rectangular piece of septal cartilage was sutured to the underlying tip cartilages as a tip graft. 5 MONTHS POST OP.
EXTERNAL RHINOPLASTY: This patient had prominent nasal dorsum and tip. She required resection of cartilage and bone of bridge of nose, osteotomies, and lowering of tip by resecting segment of tip cartilages. Lessening of tip projection (i.e. moving tip closer to face) is best achieved through an external rhinoplasty approach.
ENDONASAL RHINOPLASTY: Entire nose made smaller with removal of very prominent bony and cartilaginous hump.
ENDONASAL RHINOPLASTY: Bony and cartilaginous hump removed, entire nose lowered closer to face and tip rotated upwards.
EXTERNAL RHINOPLASTY: This patient had a significant hump reduction. There is still slight fullness above tip which will improve further over the coming year but already very pleasing profile. 4 MONTHS POST OP.
EXTERNAL RHINOPLASTY: This involved osteotomies and bony and cartilaginous hump reduction. She also underwent partial resection of the tip cartilages to improve tip definition. 4 MONTHS POST OP.
EXTERNAL SEPTORHINOPLASTY: This patient underwent External septorhinoplasty surgery – This was quite involved given the degree of reduction necessary. Surgery included resection and suturing of tip cartilages with thinning of overlying skin and bony and cartilaginous hump reduction and ostetomies. 21 MONTHS POST OP.
EXTERNAL SEPTORHINOPLASTY: Given the degree of reduction and thickness of skin she required several steroid injections post op. 21 MONTHS POST OP.
REVISIONAL RHINOPLASTY: This patient underwent primary rhinoplasty surgery elsewhere, In retrospect there was overresection of dorsum and boxy squarish tip. With revision rhinoplasty, the dorsum was raised and the tip contour was modified using multiple grafts harvested from her ear cartilage (conchal cartilage).
EXTERNAL RHINOPLASTY: This patient had marked over projection of the tip and a prominent hump primarily comprised of cartilage. Significant resections of several components of the tip cartilages were necessary to move the tip closer to the face. This is best done through an external rhinoplasty approach.
ENDONASAL RHINOPLASTY: Hump reduction
ENDONASAL RHINOPLASTY: This patient had a moderate bony and cartilaginous hump deformity with normal tip position. The cartilaginous and bony hump was removed and the tip was refined slightly.
ENDONASAL RHINOPLASTY: This patients main issue was of a very bulky rounded tip. This is related to both prominent tip cartilages and thick skin. Surgery involved not only removing and sculpting tip cartilage but also removing soft tissues between the cartilage and skin.
EXTERNAL RHINOPLASTY: This patient had an unusual issue with collapse of the tissues lateral to (at each side of) the tip of her nose. On the profile you can also see that she had a very mild convexity or (nasal hump) which was lowered. In addition, the nasal tip was rotated upwards slightly and refined.
EXTERNAL RHINOPLASTY: This patient had an unusual issue with collapse of the tissues lateral to (at each side of) the tip of her nose. This was treated surgically with placement of cartilage taken from the inside of her nose which was then placed atop her tissues in this area.
EXTERNAL RHINOPLASTY: This patient had overprotection of his nasal dorsum and tip. He underwent External rhinoplasty to not only straighten his profile but significantly reduce the size of his nose. ONE YEAR POST OP.
EXTERNAL SEPTORHINOPLASTY: This patient had a prominent cartilaginous dorsum (i.e. just above the tip) which was lowered along with upward rotation of the tip and slight flattening of the hanging columella. This was combined with tip narrowing techniques i.e. partial resection of the tip cartilages as well as suturing of the tip cartilages to improve the symmetry and narrowness of the tip. 5 MONTHS POST OP.
EXTERNAL SEPTORHINOPLASTY: This patient had a prominent cartilaginous dorsum (i.e. just above the tip) which was lowered along with upward rotation of the tip and slight flattening of the hanging columella. This was combined with tip narrowing techniques i.e. partial resection of the tip cartilages as well as suturing of the tip cartilages to improve the symmetry and narrowness of the tip. 5 MONTHS POST OP.
EXTERNAL RHINOPLASTY: This patient had multiple issues. She had a nasal hump but also a saddle deformity (i.e. above the tip was a deficiency of cartilage). Through an external rhinoplasty the hump was lowered and the tissues above the tip augmented using the patient’s own cartilage. 1 MONTH POST OP.
EXTERNAL RHINOPLASTY: On the front view one can appreciate a narrowing of the tip. This picture was taken just one month following external rhinoplasty and further tip narrowing will occur as the tissues heal. 1 MONTH POST OP.
ENDONASAL RHINOPLASTY: This patient had a large nasal hump deformity. Through closed or endonasal rhinoplasty, cartilage and bone was removed to lower the dorsum. Osteotomies also performed.
RHINOPLASTY WITH CHIN AUGMENTATION: Patient underwent removal of bony and cartilage hump and upward rotation of nasal tip. Often patients requesting rhinoplasty also benefit from augmentation of the chin to enhance the profile further. A chin implant was placed through the mouth with no external scars.
ENDONASAL RHINOPLASTY: This patient had marked nasal hump and downturned tip with excess nostril showing. She underwent removal of bony and cartilage hump with rasping as well as osteotomies. Also significant upward rotation of tip and slight narrowing of tip with resection of tip cartilages in 2 locations.
